s Choose this packet if you have, or will require a non-immigrant alien visa for entry into the United States Application Packet for International Students
s Choose this packet if you are a citizen of the United States or have been admitted to the United States as a Resident Alien Application Packet for Domestic Students

General Instructions

  1. Open the appropriate file. Print the complete file. Make two(2) additional copies of the last page(reference form) since you will need to submit three(3) references. Read all instructions carefully before completing the forms (The instruction pages do not have to be returned). Complete all forms in ink or by typing, sign, and date where required.
